Evidence-Based Medicine for Pharmacists in the Patient-Centered Medical Home (PCMH)

The medical home model holds promise as a way to improve health care in America by transforming how primary care is organized and delivered. Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care. To support informed health care decisions by patients and their pharmacists in the PCMH and to improve the quality, effectiveness, and efficiency of health care through evidence-based practice, AHRQ developed this CPE activity entitled "Evidence-Based Medicine for Pharmacists in the Patient-Centered Medical Home (PCMH)."

Learning Objectives

After participating in this activity, the pharmacists will be able to:

  • Define the tenets of the patient-centered medical home and AHRQ’s role.
  • Describe the various patient-centered medical home models and list the numerous roles for pharmacists in a PCMH.
  • Discuss successful implementation strategies and potential barriers to the PCMH.
  • Recognize the Effective Health Care Program as an evidence-based resource for pharmacists.

Continuing Pharmacy Education Credit

The American Pharmacists Association (APhA) and the Agency for Healthcare Research and Quality (AHRQ) have collaborated for the purpose of developing and providing continuing pharmacy education for the activity entitled “Evidence-Based Medicine for Pharmacists in the Patient-Centered Medical Home (PCMH).”

ACPEAPhA is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. The ACPE Universal Activity Number assigned to the activity by the accredited provider is: 202-999-10-270-H04-P. This activity is approved for 1.5 contact hours of continuing pharmacy education credit (0.15 CEUs).

To obtain continuing pharmacy education credit for this activity, participants must participate in the entire activity, and complete the online CPE examination and activity evaluation located on www.pharmacist.com Exit Disclaimer by December 13, 2013. A Statement of Credit will be automatically generated upon achieving a passing score of 70% or better. There is no fee to participate in this activity.

Presenter Biosketches

Sarah J. Shoemaker, Pharm.D., Ph.D.

Dr. Sarah J. Shoemaker is a health services and policy researcher with Abt Associates, Inc. in Cambridge, Massachusetts. Dr. Shoemaker has conducted health services and policy research and evaluations on behalf of several federal health agencies including AHRQ, CDC, CMS, and HRSA. Dr. Shoemaker has conducted studies across several areas of interest including: pharmacy practice, implementation research, quality improvement, health literacy, health IT, and medication therapy management (MTM). For example, she has led studies to develop a rating system for assessing patient education materials linked to EHRs; assess health literacy practices in retail pharmacies; understand quality improvement efforts in pharmacy; examine MTM programs for Medicare Part D; develop measures of health IT adoption and use; explore comparative effectiveness research opportunities; and examine retail clinic business models, services, vaccines, and patients’ perceptions. Additionally, in collaboration with pharmacy colleagues, Dr. Shoemaker has defined and continued to examine the value of patients’ medication experiences (or the meaning of chronic medications for patients) in effective medication management, as well as patient-centeredness in pharmacy practice. Dr. Shoemaker is a registered pharmacist who received her Pharm.D. from Creighton University in Omaha, Nebraska and her Ph.D. in Social and Administrative Pharmacy from the University of Minnesota. She has published in the peer-reviewed pharmacy literature and presented at several pharmacy and health policy conferences. Finally, Dr. Shoemaker is an Adjunct Assistant Professor of Pharmaceutical Sciences at the Massachusetts College of Pharmacy and Health Sciences, serves as a peer reviewer for several journals, and Editorial Board Member for the peer-reviewed journals: Research in Social and Administrative Pharmacy and INNOVATIONS in Pharmacy.

Janice L. Genevro, Ph.D., M.S.W.

Dr. Janice L. Genevro leads the Primary Care Implementation team within the Prevention/Care Management Portfolio and Center for Primary Care, Prevention, and Clinical Partnerships at the Agency for Healthcare Research and Quality (AHRQ). Dr. Genevro is a psychologist who specializes in the translation of health services research into effective practice and policy. Her research interests include patient-centered primary care, health communication, care coordination, and bereavement and loss in health care settings. She earned a doctoral degree in Health Psychology from the University of California, San Francisco. As a post-doctoral fellow at the National Institute of Child Health and Human Development, she conducted research at the intersection of child development and health. Dr. Genevro also served as a faculty member and staff researcher at the Johns Hopkins Bloomberg School of Public Health, working with other faculty and staff to evaluate programs designed to improve primary care pediatrics through the integration of child development services into routine care, and as a member of the Institutional Review Board of the National Human Genome Research Institute at NIH. Her work as a social worker focused on improving maternal and child health in families at risk.

Stephanie M. Hammonds, Pharm.D.

Dr. Stephanie Hammonds works in the Office of Pharmacy Affairs, Health Resources and Services Administration. Her work primarily focuses on the Patient Safety and Clinical Pharmacy Services Collaborative and Affordable Care Act Provisions related to the patient centered health home, care coordination during transitions of care, and clinical pharmacy services. Continuing to focus on these topics, she hopes to obtain a PhD in Health Services Research, Economics, and Policy. In September 2008, she was appointed as a Congressional Health Policy Fellow and obtained placement in the U.S. Senate Committee on Health, Education, Labor, and Pensions Majority Health policy office, for Senators Kennedy and Harkin, where her work focused on Quality and Delivery System improvement. Although she contributed to other sections as well, she wrote Section 3503 of the Affordable Care Act (PL 111-148, PL 111-152), Medication Management in the Treatment of Chronic Diseases which was unanimously accepted as a “non-controversial” amendment to H.R. 3962 by the U.S. House of Representatives Committee on Energy and Commerce. Dr. Hammonds earned her Doctor of Pharmacy degree in 2004 from Purdue University.

Karen Williams, Pharm.D., M.B.A.

Dr. Karen Williams is a Branch Chief for Quality Improvement (QI), Office of Pharmacy Affairs, at the Health Resources and Services Administration. Dr. Williams manages clinical and quality initiatives that promote innovative opportunities and partnerships with internal and external stakeholders. One of the primary responsibilities of the QI branch is to manage the Patient Safety and Clinical Pharmacy Services Collaborative. She has a personal interest in promoting best practices in pharmacy and mentoring future pharmacy leaders. Before beginning her federal career she practiced for almost a decade in community hospital and ambulatory pharmacy care settings. Dr. Williams is an active member of the Public Health Service Rapid Deployment Force pharmacy team. She has been with HRSA’s Office of Pharmacy Affairs (OPA) for over five years, and is a 340B Drug Pricing Program expert. Dr. Williams received her Doctor of Pharmacy from Howard University and a Master of Business Administration from Georgetown University.

Vince Willey, Pharm.D.

Dr. Vincent J. Willey is an Associate Professor of Pharmacy at the Philadelphia College of Pharmacy, University of the Sciences in Philadelphia. Prior to this position, he was the Vice President of Research for HealthCore, a health outcomes research company located in Wilmington, DE. He received his Bachelor of Pharmacy degree from the Philadelphia College of Pharmacy and Science and his Doctor of Pharmacy degree from the Philadelphia College of Pharmacy at the University of the Sciences in Philadelphia. Dr. Willey has published over 25 original research manuscripts in peer reviewed journals and has presented research at numerous medical, pharmacy and research national meetings. He has also served as a peer reviewer for several medical and pharmacy journals and is currently a member of the Journal of Managed Care Pharmacy’s Editorial Advisory Board. His research interests are focused on the outcomes associated with medication therapy in the “real-world” setting of clinical practice, the therapeutic area of dyslipidemia management and the pharmacist within the medical home concept. Dr. Willey’s teaching responsibilities include several therapeutic areas within the Pharmacotherapeutics course, biomedical literature evaluation and presentation skill development within the Professional Seminar course and use of statistics by clinicians. In addition, he maintains a clinical practice within an outpatient family medicine setting and serves as the faculty advisor for the Academy of Managed Care Pharmacists (AMCP) University of the Sciences student chapter.

Scott R. Smith, R.Ph., M.S.P.H., Ph.D.

Dr. Scott R. Smith is a Senior Service Fellow in the Center for Outcomes and Evidence (COE). Dr. Smith is a registered pharmacist with a Master's degree in public health from the University of Illinois and a doctorate in pharmaceutical sciences from the University of Michigan. At AHRQ, Smith heads the pharmaceutical outcomes research programs involving collaborations across all AHRQ centers and programs. In the COE, Dr. Smith works with the Centers for Education and Research on Therapeutics (CERTs), the Evidence-based Practice Centers (EPCs), Research Initiative in Clinical Economics (RICE), and the technology assessment group. Dr. Smith's research interests are related to pharmaceutical outcomes and he is a national expert in designing studies that measure medication adherence/compliance. Dr. Smith's other research interests include pharmacoepidemiology, access to health care services, geographic information systems, health care informatics, social-behavioral risk factors, the adverse effects of chronic drug exposure, and longitudinal data analysis.
